A new train being tested overran the track and ended up in the dirt. The irony is, back in 1972, BART premiered with the Fremont Flyer incident https://www.flickr.com/photos/walkingsf/8143196966


The difference here is BART crashes them before putting them into service WMATA it does after.

What I find puzzling is, why didn't the train go into fail safe mode when hydraulic brake pressure dropped below certain threshold.

There's a fuller explanation in the California forum--it had to do with its being a single-car movement; if it had been in a train of 6 cars or so the other cars would have braked.

It still shouldn't happen on a single car basis. The fail-safe of loss of power should be brakes applied.

Rohr Industries that built Metro's unsafe 1000 series car also built cars for BART.

The 1 k cars are no more or less safer the any other cars in WMATA fleet, including the 7k cars. The game plan at the time WMATA and BART were being designed and built was to put in place a robust signaling and train control system that would prevent collision. In WMATAs case said signaling and train control system was not properly maintained to designed specifications.

The difference between rail operations in the United States compared Europe is: United States, build them heavy and indestructible to withstand collision. Europe, build them light and put in place a signaling system that prevent collision.
