sad part is, a lot of things were not done, be it by worker or be it by his team .
despite power to be called off the circuit must be grounded as off does not mean no power due to induction . second someone in team should have done a power reading (stick ) to see if circuit was actually dead, it seems as rules were violated with disastrous result specially at this time of year.
