I went to Google News, used keywords czech train, and came up with several reports, including this one from the Telegraph (U.K.):

According to the Telegraph, this crash into a fallen bridge happened in the Czech Republic, a bit southwest of Ostrava. The site appears to be about 30 km from Poland.
