At about 6:12 PM, EXTRA 2985 WEST ( 70 cars + caboose) while moving at about 50 mph through OD Interlocking on Tk. 1 derailed the front truck of the 32nd head car on the diamond with Tk. 2 of the Youngstown Branch, followed by a general derailment of 14 cars at the frog of the facing point 1 to 2 xover. Derailed equipment obstructed Tk. 3.

EXTRA 2978 WEST (75 cars + caboose) moving at 40 mph on Tk.3 ran into the derailed cars derailing the 2978, tender, and the 15 head cars, some of which struck and derailed cars on the adjacent yard track. The Engr. was killed and the fireman and Head Brakeman were injured when the cab was crushed.

The rear end crew of the first train gave a washout signal, and the Engr. of 2978 dumped the air but there was insufficient distance to stop before impact.
