Some of the biggest incidents were out of NICTD control (the semi losing it's load off the toll road in Gary, NIPSCO's high voltage line falling on NICTD's lines near Furnessville, and everything wrong on Metra). They have had their fair share of equipment malfunctions in the nastiest weather.

I don't know if the recent red signal incidents were caused by vandals but there were some reported last year to be caused by people (using the term loosely) cutting wires off of the track. Not cool.
