NS Georgia/Alabama Track Authority Forms

Hello,
Does anyone know where I can get a NS Track Authority form? I copy down the info. during the call outs between the Dispatch and train crews before I head out trackside to get a pic. It makes a nice record to go back and review whenever I need to. Just recently the Alabama Div. started using the new forms that the Georgia Div. has been using for a couple of years now. The old forms I have are no longer any good as the new ones are laid out differently than the old.

Can't help out, but my suggestion is that when you do get a copy, scan it into your comp, and save the scan, allowing you to print out as many as you need for later use.

If I can ever get my hands on one that's exactly what I'm going to do J.D.
