The thing that usually kills people in fire is smoke inhalation. It can happen very quickly and the crewmembers probably didn't have enough time to get out alive.

The gears in a locomotive are on the traction motors and on the wheels that the traction motor gear meshes with. Most likely the cover on the traction motor came loose and caused the trouble with the switch.

First off, that looked like bit more than just a 10ft fall.... try 20+ft.

Second, there are no such things as seat belts, air bags, crumple zones etc etc etc in locomotives. A fall like that would at the very least severely injure the crew, either from the shock of impact or flying about the cabin.

Throw fire into the mix and you got yourself a sticky situation.
