Double-track, TCS Rules.

At about 5:55 AM, X6474 EAST (2 GP-9's / 111 cars [76 x 35] / 8023 tons) and X4367 WEST (2 GP-40's / 42 cars / 3430 tons) both approached the CP at Orleans Rd. on Tk. 2; both had "Approach" and "STOP" signals. Both on Tk. 2 as the Dispatcher had Amtrak #32 on Tk. 1 and intended to have X4367 WEST cross to Tk. 1 after #32 passed.

X 6474 EAST failed to comply with the restrictive signals and ran through the CP at about 38 mph, entered a right-hand curve, and collided head-on with X4367 WEST which was moving at about 20 mph. The female Fireman who was operating the X 4367 was killed and the other head-end crewmen were injured. 4 units and about 20 cars were derailed; damage est. was $1,700.000.
