EXTRA 6741 WEST (125 mty cars + caboose) had stopped en route to add helper engine 6810 facing east on the rear end because the 6741 was not steaming well.

Entering the Yard at RV, the train parted between the 51st and 52nd head cars. The Conductor and Flagman were on the front platform of the caboose preparing to cut off on the fly and had closed the angle cocks on the rear of the 125th car and the caboose. The 6810 was still pushing when the emergency application occurred. The 125th car overrode the caboose and the two trainmen were killed.
