In this nighttime accident, an Ashtabula city streetcar approached the level xg of the LS&MS / PRR tracks, and the xg gates were not lowered. The streetcar proceeded across the tracks and was t-boned by a northbound LS&MS coal train. When the engineer dumped the air just before impact, the train buckled and buried the wreckage of the streetcar under coal. Final count was 8 dead and 5 injured.

After this, there was an underpass constructed.
