Lehigh Valley Railroad Company ETT No. 9 dated 27 October 1969, Special Instructions 1158-E, page 71 entitled Pusher Engines Commonwealth of Pennsylvania deals only with the crew vacating the cab when helper power exceeds 3500 hp and also with not cutting off on the fly if helper hp exceeds 3500 hp.
ETT No. 10 dated 10 August 1975, Special Instructions 1158-F page126 gives the same information as 1158-E above. In addition, SI 1158-H, page 127 entitled Use of Helper Locomotives limits helper consists to twelve axles and also not to exceed the number of axles in the leading power consist. The SI further restricts two 6-axle and three 4-axle helper consists to 6 throttle except when shoving coal trains.
The above are the only reference to helpers that I can find in LVRR ETTs dating back to 1952.
