Located on the Sandusky Division of the LE&W District between Lima, OH and Frankfort, IN.

Single-track with TT, TO's, and MBS and Yard Limit rules.

Train #55 (Local Freight) Arv'd. at about 2:45PM and commences switching. At one point, this required the use of the Strawboard Track as well as the main track.
The Cond. positioned himself near the Strawboard switch.

Train 2nd #47 with Eng. 701 and 65 cars plus caboose failed to comply with the Yard Limit Rules and approached at about 45 mph. No brake application was made in the vicinity of the NYC xg located about 1/2-mile before the start of a 20mph speed restriction.

The Head Brakeman called a warning that the Strawboard switch was reversed when he observed the red target at 1400' distance. The Engr. did not make an immediate emergency application and did not close the throttle. Speed was not significantly reduced when the train entered the Strawboard track and the 701 overturned killing the engine crew. 16 cars were derailed; the head brakeman jumped and was injured.
